Reach

Booked. Friday 3 Jul, 10:30 with Dr Maina. Ref KM-4821.
AI assistant
The assistant works inside the system.
It answers from the facility's own records and stays inside the clinician's workspace.
Grace Mwende is a 44 year old woman with type 2 diabetes and hypertension, followed in your chronic care clinic since 2021. Her HbA1c came back this morning at 8.9 percent, up from 8.4 in April, and she reports missing the evening metformin two or three times a week. Blood pressure is borderline on losartan at 142/92. She collects her repeat prescriptions on time.
Her open items: today's HbA1c needs a treatment decision, she is overdue for her yearly diabetic eye exam by three months, and her rejected April claim is waiting for resubmission. She is already on atorvastatin for cardiovascular risk.
For most adults with type 2 diabetes an HbA1c target of 7.0 percent is recommended. Intensify therapy when two consecutive readings exceed 8.0 percent despite adherence support, and review for insulin initiation above 9.0 percent with symptoms.
